Minced Chicken & Lettuce Soup (鶏ひき肉とレタスのスープ)

I was lazy to make meat ball. In stead, I didn't rigorously stir minced meat so that it forms some chunks.
Combination of chicken, lettuce, and mushroom is quite nice.
